Requirements

  • Microchip recorded (ISO 11784/11785)
  • Rabies vaccination (≥2 lifetime doses)
  • Rabies antibody titer (OIE-FAVN)
  • Identity attestation issued

Owner checklist

  • CVI certifying statement: New World Screwworm freedom Required

All animals requiring a CVI (Certificate of Veterinary Inspection) for importation into Hawaii must have a certifying statement: "All animals were inspected and found free of evidence of New World Screwworm infestation."

  • Apply online via HIPOP Required

Submit your dog or cat's entry application online through HIPOP (https://hipop.ais.hawaii.gov/), with the required documents uploaded and the applicable fee paid by card, ahead of arrival — see the arrival-airport deadlines below.

  • Document deadline depends on your arrival airport

Landing at Honolulu: your HIPOP application and documents must be submitted no less than 10 days before your pet arrives in Hawai'i to qualify for the 5-Day-Or-Less or Airport Release programs (late submission means a higher fee — $244 instead of $185 for airport release, $130 instead of $98 for re-entry). Landing at Kona, Kahului, or Līhu'e (neighbor islands): submit 30 days or more ahead of your pet's planned arrival instead.

  • Two rabies vaccinations in the pet's lifetime Required

Hawaii requires original certificates for 2 rabies vaccinations in the pet's lifetime, AT LEAST 30 DAYS APART, the most recent given more than 30 days before arrival and still in-date (Petra's structured check enforces the count, validity, and the most-recent-dose's 30-day-before-arrival timing, but not the gap BETWEEN the two doses — confirm that from your vet's records). Each certificate must be signed in ink by a licensed veterinarian, and must list: vaccine name, lot or serial number, booster interval, vaccination date, and vaccine lot expiration date.

  • OIE-FAVN titer timing: arrive 30 days–36 months after the lab receives the sample

The clock starts the day the approved lab RECEIVES the blood sample, not the draw or result date. Too early (under 30 days) or too late (over 36 months) means quarantine.

  • Microchip must be implanted before (or at) the FAVN blood draw

DAB's own veterinarian guidance requires the electronic microchip to be implanted prior to or when blood is drawn for the OIE-FAVN test, with the microchip number recorded on the lab submission form — a lab will not run the test without it. Petra enforces this ordering structurally when the microchip's implant date is on file.

  • Health certificate within 14 days of arrival Required

A licensed vet must examine the pet and issue an ORIGINAL health certificate no more than 14 days before arrival — a photocopy is not acceptable. This is mandatory for the direct-release program.

  • Tick treatment within 14 days of arrival, recorded on the certificate Required

A tick treatment must be given within 14 days of arrival and recorded on the health certificate. Revolution® is NOT an acceptable product for this treatment.

Still decided outside Petra

  • Hawaii's HIPOP entry application and its review happen outside Petra; Petra records that you submitted the application through HIPOP, not that DAB approved it.
  • The document-submission deadline (10 days for Honolulu, 30 days for Kona/Kahului/Līhu'e) is enforced by DAB at the airport, not by Petra; missing it changes your program/fee but Petra cannot verify timely submission itself.
